OK, now God has called you  --  it's time to do the paperwork!

Sleeper Registration, Medical and Permission

All teenagers under 18 years old are required to have their parent's permission. A medical information form is also required.  Because a parent signature is needed, these forms must be completed in hard copy.  You may turn the form into your sponsor or mail it to the Awakening address.

Sponsor's Form

Share your Awakening experience and pass the love of Jesus onto friends, family, and those God has put in your life.  Each sponsor must complete a sponsor's form and return it to a team member or mail it to the Awakening address.  Be sure to include your guest's name.  You are encouraged to include the sponsor’s fee with the paper work.

Sponsor's have other responsibilities for their sleepers on the weekend.  If you are unsure of the sponsor's role, email your questions to bryanawakening@bryanawakening.org and we will send you more information.

Volunteer

Let us know if you would like to serve on a weekend and serve the Lord through this ministry.  It takes the effort of many people to facilitate these weekends.  Please mail the form to the Awakening address.
